Members of our community specifically told us there was a need for more support connecting seniors and individuals on Medicaid with services that may be available to help them remain safe, comfortable and independent at home.

They also told us the process can be confusing.

We listened and we answered the call by creating the Help at Home Program.

We’ll meet with you in person, at no cost, learn about the help you or your loved one may need, and help you understand the next steps for exploring Medicaid-supported services.
